Job summary

Beyond Home Health Care, a NJ DDD-approved provider, seeks a licensed Physical Therapist to deliver in-home and community-based PT services to adults with developmental disabilities across New Jersey.

You will evaluate mobility, strength, balance, gait, and functional movement, and implement person-centered interventions to maximize independence and community participation, aligned with NJ DDD guidelines.

Qualifications

• NJ licensed Physical Therapist (PT) in good standing.
• Graduate of a CAPTE-accredited PT program (DPT or equivalent).
• Minimum 1 year clinical experience preferred; new grads considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.

Responsibilities

• Conduct comprehensive in-home and community-based PT evaluations and develop individualized treatment plans.
• Deliver one-on-one therapy sessions in participants' homes and community settings.
• Document services per DDD standards (notes, progress, outcomes) within required timeframes.
• Collaborate with support coordinators, families and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Adhere to DDD policies, HIPAA, and mandated trainings.
• Maintain accurate billing records per DDD rates and authorizations.
