Job summary

Beyond Home Health Care is seeking a licensed Physical Therapist to deliver in-home and community-based PT services to adults with developmental disabilities in New Jersey. You will evaluate mobility, strength, balance and functional movement, and implement person-centered interventions to maximize independence and community participation.

Qualifications

• NJ license as a Physical Therapist (PT) issued by the NJ State Board of Physical Therapy Examiners.
• Graduation from a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver’s license, and current auto insurance.
• Ability to pass DDD background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy services in the participant’s home and community settings.
• Document services in compliance with DDD standards and deadlines (session notes, progress notes, outcomes).
• Collaborate with Support Coordinators, families, caregivers and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce therapeutic goals between sessions.
