Job summary

Beyond Home Health Care is a NJ DDD-approved provider that delivers in-home pediatric and adult PT services to individuals with developmental disabilities statewide. The role focuses on in-home assessments, mobility training, and community participation, following NJ DDD guidelines and ISP goals.

The licensed PT will evaluate mobility, strength, balance, gait, and functional movement, delivering person-centered interventions to maximize independence and reduce secondary complications.

Qualifications

• Current New Jersey PT license
• Graduate of CAPTE-accredited PT program (DPT or equivalent)
• Minimum 1 year of clinical experience preferred; new graduates considered
• Current CPR/BLS certification
• Clean background check and fingerprinting per NJ DDD requirements
• Reliable transportation, valid NJ driver’s license, and current auto insurance
• Ability to pass DDD background checks including CRoff

Responsibilities

• Conduct comprehensive evaluations and develop ISP-aligned treatment plans.
• Deliver one-on-one therapy services in home and community settings.
• Document services per DDD standards with notes, progress notes, and outcome measures.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals.
• Adhere to DDD policies, HIPAA confidentiality, and related laws.
• Maintain accurate billing records per DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle’s/Stephen Komninos’ Law, etc.)
