Job summary

Powerback Rehabilitation to You in Rochester, NH is seeking a full-time Physical Therapist to provide in-home outpatient rehabilitation for older adults, delivering care in patients' homes or in independent living facilities.

You will assess, plan, and implement individualized therapy programs, educate patients and caregivers, and coordinate with the rehabilitation team to promote independence, safety, and optimal quality of life.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy (or PT-related field).
• Bachelor’s degree in Physical Therapy with 5 years of progressive PT experience.

Responsibilities

• Provide in-home outpatient PT services to older adults.
• Assess patient needs, develop and implement therapy plans.
• Educate patients and caregivers on exercises and self-management.
• Maintain documentation and billing records for services.
• Collaborate with the rehabilitation team to optimize care.

Education

Master’s degree in Physical Therapy

Master’s degree in PT-related field + Bachelor’s in PT

Bachelor’s degree in Physical Therapy + 5 years of PT experience
