Job summary

Powerback Rehabilitation To You is seeking a licensed Physical Therapist to deliver outpatient rehab in clients’ homes across Florence, NJ and adjacent areas. The role emphasizes functional recovery in real-life settings with flexible scheduling and travel between patient residences.

The candidate will assess needs, design and implement PT plans, educate patients and caregivers, and collaborate with the broader rehabilitation team to maximize independence and quality of life.

Qualifications

• Must have a valid Physical Therapy license in the state of practice.
• Master’s degree in Physical Therapy or equivalent.
• Bachelor’s degree in Physical Therapy with 5 years of PT experience.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Evaluate and treat in the patient’s home or in independent living facilities.
• Develop and deliver individualized PT programs to restore function and promote independence.
• Educate patients and caregivers in skills to maintain health and prevent falls.
• Coordinate with rehabilitation team to optimize patient outcomes.
