Job summary

VNA Health Group, in partnership with Visiting Physician Services (VPS), seeks a Nurse Practitioner to deliver high‑quality, in‑home primary care to older adults who are homebound or medically complex in New Jersey.

The role, within the Parker Advanced Care Institute, focuses on comprehensive care visits, chronic disease management, telehealth, wound care and patient safety, with an emphasis on comfort and independence at home.

Qualifications

• Master’s degree in Nursing from an NLN-accredited program.
• Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEA.
• National certification as an Adult or Geriatric Nurse Practitioner.
• 2+ years of APN experience and/or significant RN experience.
• Excellent communication, organizational and interpersonal skills.
• Strong computer skills.

Responsibilities

• Provides high-quality care to homebound patients.
• Assesses, diagnoses and plans care for homebound patients.
• Monitors patient condition and response to treatment.
• Represents the agency with patients, families and other health entities.
• Participates in QAPI and in-service programs.
• Follows infection control and safety policies.
