Job summary

Qualifications

• A valid driver's license, auto insurance, and reliable transportation are required.
• Ability to leverage technology and EMR systems for documentation.
• Basic Life Support (BLS) certification is required.

Responsibilities

• Travel to patients’ homes within the assigned service area.
• Provide patient-centered, low-acuity in-home visits focused on preventive care services.
• Collect and manage samples, ensuring proper labeling and lab submission.
• Perform diagnostic checks (temperature, pulse, blood pressure).
• Document care provided and interactions with patients, caregivers, doctors, and staff clearly.
• Uphold infection control practices and maintain a clean, safe environment.
