CenterWell Home Health is seeking a Preventive Care Coach Home Health Aide to perform in-home preventive care visits across the service area. You will travel to patient homes, perform low-acuity visits focused on preventive services, collect samples, and manage them with proper labeling and lab drop-off.

You will conduct basic diagnostic checks and document care thoroughly to ensure continuity, while upholding infection control standards for a clean, safe environment.

Formación

• A valid driver's license, auto insurance, and reliable transportation are required.
• The ability to leverage technology and EMR systems for documentation.
• Basic Life Support certification.

Responsabilidades

• Travel to patients' homes throughout your assigned service area.
• Deliver patient-centered, low acuity in-home visits targeting preventive care services.
• Sample collection and management: ensure samples are labeled properly and dropped off at the appropriate lab.
• Diagnostic checks: temperature, pulse and blood pressure measurements.
• Document the care rendered to the patient to ensure continuity of care; document interactions with patients, caregivers, doctors and other staff members.
• Practice acceptable infection control principles to facilitate a clean, safe and comfortable environment.
