Job summary

Signify Health is hiring a full‑time physician to perform in‑home health evaluations for health plan members. You will visit patients in their homes, review medications and medical history, and provide comprehensive evaluations using an iPad-based workflow.

Travel locally within licensed states and, when needed, conduct virtual visits. The role emphasizes building trusted relationships, improving health outcomes, and coordinating care with health plans and PCPs.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide In-Home Health Evaluations and engage with patients in their homes.
• Bridge gaps in care and connect members with the right care.
• Travel locally within licensed states as needed; conduct virtual visits when required.
