Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).
• MDs and DOs welcome and eligible to practice in home-health evaluations.

Responsibilities

• Provide In-Home Health Evaluations to patients in their homes.
• Engage with patients and families to review medications and medical history.
• Coordinate with health plans and care teams to connect patients with appropriate services.
• Travel locally and within licensed states to conduct visits.
• Conduct virtual visits when needed and document findings.
• Utilize iPad and clinical workflow to record assessments.
