Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to deliver in-home health evaluations for members in their own homes, bridging gaps in care. You’ll connect with patients, review medical histories, perform physical assessments, and optionally coordinate with health plans through a digital workflow.

The role requires MD or DO credentials, unrestricted licensure, and the ability to travel locally within licensed states.

Qualifications

• Active, unrestricted medical license in the coverage area.
• Multi-state licenses preferred.
• MD or DO degree required.

Responsibilities

• Provide in-home health evaluations to patients in their homes.
• Engage with patients, review medical history, and perform physical evaluations.
• Occasionally conduct virtual visits and coordinate with health plans.
• Travel locally within licensed states as needed.
