Signify Health, part of CVS Health, seeks a Per Diem Physician to provide in-home health evaluations, engaging with patients in their homes and bridging gaps in care. The role involves visiting members nationwide, delivering personalized health assessments, and coordinating with care teams.

The position supports 12 weekly hours (part-time) with a competitive hourly pay and full benefits for eligible hours. Travel to licensed states is required, and remote or virtual visits may be needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsabilités

• Visit patients in their homes as part of Signify Health's clinician network.
• Conduct in-home health evaluations using an iPad workflow; communicate findings clearly.
• Provide Diagnostic & Preventive Services as indicated by health plan.
• Travel locally and within licensed states as needed.
• Perform virtual visits when required.
• Build trusted relationships to help connect patients with the right care.
