Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide in-home health evaluations for members in their own homes, offering flexible, personalized care. Visit length is about 45 minutes and involves history review, exam, and plan coordination.

The role requires active MD/DO licensure, ability to travel locally and within licensed states, and the option to conduct virtual visits as needed.

Formación

• MD or DO with active, unrestricted license(s).
• Multi-state licenses preferred.
• Experience performing in-home health evaluations is a plus.

Responsabilidades

• Conduct in-home health evaluations lasting about 45 minutes.
• Review medication history and medical information.
• Perform physical evaluation and, if required by the health plan, arrange additional tests.
• Travel locally and within licensed states as needed.
• Occasionally conduct virtual visits.
