Job summary

Signify Health, part of CVS Health, seeks a Per Diem Physician to perform in-home health evaluations at patients' homes. You will conduct visits that include medication and medical history review, physical examination, and coordination with health plans.

This part-time role requires MD or DO licensure with unrestricted status and may involve local travel within licensed states. Bilingual or multi-state licensing is a plus.

Qualifications

• Active, unrestricted medical license in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations during 45-minute visits.
• Review medications and medical history, perform physical assessment.
• Coordinate with health plans to provide appropriate care pathways.
• Travel locally and within licensed states as needed.
• Option to conduct virtual visits when required.
