Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide In-Home Health Evaluations, visiting patients in their own homes and bridging gaps in care. You will conduct 45‑minute visits, review medications and medical history, perform a physical eval, and coordinate with health plans for recommended services, while traveling locally across licensed states as needed.

Qualifications

• Active, unrestricted license in coverage area(s).
• Multi-state licenses preferred.
• Prior success delivering in-home health evaluations or similar roles is a plus.

Responsibilities

• Provide In-Home Health Evaluations, visiting patients in their homes.
• Conduct 45-minute visits including medication/medical history review and physical evaluation.
• Coordinate with health plans and care teams to connect patients with appropriate services.
• Travel locally across licensed states as needed; conduct virtual visits when required.
