Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide in-home health evaluations. The role involves visiting members in their homes, reviewing medications and medical history, and performing a physical assessment with optional diagnostic tests as dictated by the health plan.

Flexible to travel locally and within licensed states; virtual visits may be required. Prior in-home or field-based physician experience is a plus, with a focus on compassionate, patient-centered

Qualifications

• Active, unrestricted medical license(s) in coverage area(s)
• Multi-state licenses are preferred
• Experience delivering in-home health evaluations is a plus

Responsibilities

• Provide in-home health evaluations lasting about 45 minutes
• Conduct medication and medical history review during visits
• Perform physical evaluations and recommend additional tests per health plan orders
• Utilize iPad and clinical workflow to document evaluations
• Travel locally and within licensed states as needed
• Possibly conduct virtual visits when required
