Job summary

Signify Health, part of CVS Health®, is seeking a Per Diem Physician to provide in-home health evaluations. This part-time role involves visiting patients in their homes, conducting health questions, history and physicals, and potential diagnostic testing as per health plan orders.

Ideal candidates hold an active MD or DO license, with multi-state licenses preferred, and are willing to travel locally within licensed states or perform virtual visits as needed.

Responsibilities

• Conduct in-home health evaluations (about 45 minutes) and review medication/medical history
• Perform physical evaluations and may order diagnostic tests as needed by the health plan
• Travel locally within licensed states and conduct virtual visits when required
