Qualifications

• Active, unrestricted MD or DO license in coverage area.
• Multi-state licenses preferred.
• Ability to travel locally and within licensed states.

Responsibilities

• Conduct in-home health evaluations for members.
• Review medication and medical history and perform physical evaluation.
• Conduct virtual visits as needed.
• Provide all Signify Health services (Diagnostic & Preventive Services).
• Collaborate with health plans to bridge gaps in care.
• Maintain documentation and HIPAA-compliant records.
