Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to perform in-home health evaluations for members in their own residences, connecting patients with appropriate care. The role supports flexible travel within licensed states and requires MD/DO licensure with multi-state capability.

The position offers a 12-hour weekly schedule, part-time hours, and a pay range reflecting base hourly rates plus incentives.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Visit patients in-home for health evaluations (~45 minutes) to review medications and medical history, perform a physical evaluation, and coordinate with health plans for additional tests as ordered.
• Join a national network of clinicians delivering in-home health evaluations and connecting patients with the right care.
• Contribute as a Signify Health clinician, fulfilling the role requirements and traveling locally as needed.
• Conduct virtual visits when required to support patient care.
• Provide diagnostic and preventive services such as spirometry, bone density, and related tests per program guidance.
