Job summary

Signify Health, part of CVS Health®, is seeking a Part Time Physician to deliver In-Home Health Evaluations. You will assess health needs in patients’ homes, review medications and medical histories, and perform physical evaluations using an iPad and streamlined workflow.

A flexible schedule and nationwide travel support your commitment to personalized care. The role emphasizes being a field clinician with certification in MD/DO, active licensure in coverage areas, and the ability to visit

Qualifications

• MDs/DOs with active, unrestricted license(s) in the coverage area.

Responsibilities

• Provide in-home health evaluations with patient-centric, evidence-based care.
• Build trusted relationships to connect patients to the right care.
• Collaborate with Signify Health clinicians to coordinate care.
• Travel locally within licensed states and conduct virtual visits as needed.
• Perform diagnostic and preventive services as indicated by patients.
• No prescribing, lab orders, or changes to current treatment without plan.
