Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations to members in their homes. You will perform a comprehensive physical, review medical history, and coordinate care with health plans.

The role involves visiting patients at home, documenting findings via an iPad workflow, and offering diagnostic and preventive services as needed. Travel within licensed states is required, with virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Conduct in-home health evaluations and build trusted relationships with members.
• Engage with patients and families to bridge gaps in care and tailor health assessments.
• Utilize Signify Health services including diagnostic and preventive screenings as ordered.
• Travel locally and within licensed states as required; perform virtual visits when appropriate.
• Use iPad-based clinical workflow to document findings and health information.
• Support the patient’s health plan with referrals and care coordination.
