Job summary

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Conduct in-home visits with health plan members to review medical history, perform physical evaluations, and provide diagnostic/ preventive services as indicated.
• Engage patients and families to connect them with appropriate care and resources.
• Travel locally and within licensed states as required; conduct virtual visits when needed.
