Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations, visiting patients in their own homes and bridging gaps in care. The role involves 40 hours weekly, with possible local travel and virtual visits where needed, using iPad and clinical workflow.

Responsibilities include a 45-minute visit, history review, physical evaluation, and optional diagnostic tests as per health plan orders. Comprehensive benefits are provided for full-time employees.

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Multi-state licenses (preferred)

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
