Signify Health is seeking a full-time physician to provide in-home health evaluations, engaging with members in the comfort of their own homes. This role focuses on bridging gaps in care and building trusted relationships to improve health outcomes.

You will travel locally and within licensed states, conduct virtual visits as needed, and provide comprehensive diagnostic and preventive services in collaboration with the care team.

Qualifications

• Active, unrestricted MD/DO license in coverage area(s).
• Multi-state licenses are preferred for broader coverage in multiple states.
• Physician with clinical in-home evaluation experience is expected.

Responsabilités

• Provide in-home health evaluations with patient-centered care.
• Build trusted relationships to improve health outcomes.
• Collaborate with Signify Health clinicians and care teams.
• Travel locally and within licensed states as needed for visits.
• Conduct virtual visits when required by the plan or member.
• Provide recommended Signify Health services, including diagnostic and preventive screenings.
