Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide in-home health evaluations, engaging with patients in their homes to bridge gaps in care. You will build trusted relationships and deliver comprehensive health assessments using an iPad and standardized workflows.

This role includes travel within licensed states and occasional virtual visits as needed. The position requires MD or DO with active licensure, multi-state licenses preferred, and a full-time schedule with

Qualifications

• Active, unrestricted medical license in the applicable coverage area(s).
• Multi-state licenses are preferred.

Responsibilities

• Conduct in-home health evaluations with patients, reviewing medications and medical history.
• Perform physical assessments and provide diagnostic and preventive services as ordered by health plans.
• Collaborate with Signify Health clinicians to coordinate care and referrals.
• Travel locally within licensed states as needed and conduct virtual visits when required.
• Utilize Signify Health workflows and devices to deliver quality evaluations.
