Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Physician to deliver in-home health evaluations in patients' homes, connecting members with appropriate care. The role focuses on medication and history review, physical examination, and coordination with health plans, without prescribing or modifying treatment.

The position requires active medical licensure, the ability to travel locally and within licensed states, and participation in a comprehensive benefits package.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations and bridge gaps in care.
• Build trusted relationships with patients and families.
• Conduct evaluations using clinical workflow and tools without prescribing or altering treatment.
• Travel locally and within licensed states as needed to visit members.
• Perform virtual visits when required.
• Deliver Diagnostic & Preventive Services per organization guidelines.
