Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations, engaging with patients in the comfort of their own homes and helping bridge gaps in care. You’ll perform visits lasting about 45 minutes, review medications and medical history, and coordinate with health plans.

You must maintain active licenses and be willing to travel locally and within licensed states.

Qualifications

• Active, unrestricted medical license in the coverage area.
• Multi-state licenses are preferred for broader mobility.

Responsibilities

• Provide in-home health evaluations to assess medical history and current health.
• Bridge gaps in care by coordinating with the patient's health plan and care team.
• Travel within local areas and licensed states as needed, and conduct virtual visits when required.
