Signify Health is seeking a part-time physician to perform in-home health evaluations, visit members where they live, and bridge gaps in care. The role emphasizes patient-centered assessments without altering current treatments.

You will travel locally within licensed states, conduct virtual visits as needed, and participate in diagnostics and preventive services as indicated. Active MD or DO license is required and multi-state licenses are preferred.

Responsibilities

• Conduct in-home health evaluations for members
• Perform medication and medical history reviews
• Provide physical evaluations and essential diagnostics as indicated
• Travel locally within licensed states and conduct virtual visits as needed
• Collaborate with Signify Health care team to coordinate care
