Zusammenfassung

Professional Case Management is seeking a Licensed Physical Therapy Assistant to provide in-home PT services across our Home Care division. The PTA will conduct weekly visits, typically 1–2 hours per visit, and will document client progress per plan of care and agency policy.

Qualified applicants have a PTA license, at least 6 months of experience, strong communication and interpersonal skills, and a valid driver’s license with a clean record.

Qualifikationen

• Current license, registration or certification to practice as a Physical Therapy Assistant in the state(s) of employment or be eligible to take the PTA exam.
• Graduate of an accredited PTA program approved by the American Physical Therapy Association (APTA).
• Minimum of six (6) months experience as a Physical Therapy Assistant.
• Demonstrated written, verbal communication skills and interpersonal skills.
• Licensed driver with a clean driving record and insured per state/agency requirements.

Aufgaben

• Provide physical therapy services and education in clients' homes per physician Plan of Care.
• Complete visits timely and per agency policy.
• Document all services and client progress in designated forms within seven days or per policy.
• Report changes to therapist or nursing supervisor the day of change.
• Participate in interdisciplinary activities and care conferences as needed.
• Promote referrals to other disciplines when appropriate.
• Attend staff meetings and continuing education programs.
• Maintain client privacy and confidentiality; ensure safety and infection control practices.
