Job summary

Professional Case Management is seeking a Physical Therapy Assistant to conduct in-home visits for clients receiving care. The PTA will deliver therapy per the physician’s plan of care, ensuring visits are timely and well-documented.

Qualified candidates hold PTA licensure/registration, have completed an accredited PTA program, and possess strong communication skills. A valid driver’s license with a clean record is required for in-home travel.

Qualifications

• Current license/registration to practice as a Physical Therapy Assistant in applicable states.
• Graduate of an accredited PTA program approved by APTA.
• Minimum of six months PTA experience.
• Demonstrated written, verbal communication and interpersonal skills.
• Licensed driver with clean driving record.

Responsibilities

• Provide physical therapy services and education to clients in their homes following the Plan of Care.
• Complete visits timely and per Agency policy.
• Document services, observations, progress, and recommendations within seven days.
• Report changes to therapist or nursing supervisor the day of change.
• Participate in interdisciplinary activities and attend care conferences.
