Job summary

Professional Case Management (PCM) seeks a dedicated Physical Therapy Assistant to perform in-home visits, delivering therapy services as outlined by the physician's Plan of Care. You will schedule and conduct weekly visits, typically 1–2 hours per session, and document client progress accordingly.

Qualified candidates hold an active PTA license, have at least six months of PTA experience, and possess a valid driver's license with insurance.

Qualifications

• Must hold an active PTA license/registration in state(s) of employment.
• Graduate of an accredited PTA program approved by APTA.
• Minimum of six (6) months experience as a Physical Therapy Assistant.
• Licensed driver with clean driving record and valid insurance.

Responsibilities

• Provide in-home physical therapy services following the physician's Plan of Care.
• Document services, observations, and client progress within seven days.
• Maintain safety, infection control, and confidentiality in all visits.
• Collaborate with therapists, nurses, and other team members during care conferences.
