Professional Case Management is seeking a licensed Physical Therapy Assistant to perform in-home visits for clients with chronic and terminal illnesses in the United States. The role supports care teams and follows a physician Plan of Care during weekly visits.

Responsibilities include documenting services, maintaining client safety, and collaborating with therapists and nurses. In-home care is central, with strong emphasis on privacy and secure handling of patient information.

Qualifications

• Must hold current license/registration/certification to practice as a Physical Therapy Assistant.
• Graduate of an accredited PTA program approved by the APTA.
• Minimum of six months experience as a PTA.
• Demonstrated written and verbal communication skills and interpersonal abilities.

Responsabilités

• Provide physical therapy services and education to clients in their homes as per the Plan of Care.
• Complete visits timely and in accordance with agency policy.
• Document services, observations and progress and submit within seven days.
• Report changes to the therapist or nursing supervisor the day of change.
• Participate in interdisciplinary activities and staff meetings.
