Job summary

Professional Case Management (PCM) is seeking a Physical Therapy Assistant to perform in-home visits for former nuclear workers and other clients. You will follow the physician's Plan of Care, educate clients, and document progress during 1–2 hour visits weekly.

Applicants must be licensed as a PTA, graduate from an accredited PTA program, and have at least six months of experience. A valid driver’s license and driving record are required.

Qualifications

• Current license, registration or certification to practice as a Physical Therapy Assistant in the state(s) of employment or be qualified to take the PTA examination.
• Graduate of an accredited PTA program approved by the American Physical Therapy Association.
• Minimum of six months experience as a Physical Therapy Assistant.
• Demonstrated written, verbal communication skills and interpersonal skills; licensed driver with clean record.

Responsibilities

• Provides physical therapy services and education/training to clients in their homes per the Plan of Care.
• Completes visits in a timely manner and adheres to agency policy.
• Documents services provided, client progress, and recommendations within seven days or per policy.
• Reports changes to therapist/nursing supervisor promptly on the day of change.
• Participates in interdisciplinary activities and attends care conferences as needed.
• Promotes safe practice and safety/environmental assessments at client homes.
