Qualifications

• Strong interpersonal, organizational, and problem solving skills.
• Proficient with electronic medical records (EMR).

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements such as mobility, gait, balance, strength, and safety assessments.
• Develop and implement individualized care plans including manual therapy, gait training, and balance interventions.
• Continually assess and revise the PT care plan and collaborate with nursing in the multidisciplinary team.
• Educate clients and family members in rehabilitative care to promote health and independence.
• Accurately document observations, interventions, and evaluations in an EMR tablet.
