Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist to perform outpatient home health visits in Burlington, VT. This full-time Staff position serves adult and geriatric clients across Chittenden County and surrounding areas.

The ideal candidate holds an active Vermont PT license, has at least one year of clinical PT experience, and is capable of independent work. EMR skills and strong interpersonal abilities are valued assets.

Qualifications

• Licensed Vermont Physical Therapist with current license.
• Minimum of one year of recent clinical experience as a Physical Therapist.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, strength, balance, and safety assessments.
• Develop and implement individualized care plans with exercises, gait training, and other interventions.
• Assess and revise the PT care plan with nursing and multidisciplinary team.
• Educate clients and families on rehabilitative care and activities for health and independence.
• Accurately document observations and interventions in an EMR system.
