Job summary

BAYADA Home Health Care, Inc. in Vermont is seeking a licensed Physical Therapist (PT) to join our outpatient home health team in the Burlington area.

The role involves conducting visits in designated territories, developing care plans, and collaborating with a multidisciplinary team to support geriatric clients at home. Qualified candidates have a VT PT license, at least one year of relevant experience, and strong interpersonal and organizational skills; EMR experience is preferred.

Qualifications

• One year of recent clinical experience as a Physical Therapist (PT) preferred.
• Vermont PT license is required.
• Graduation from an APTA-approved PT program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform mobility, strength, balance, and gait assessments.
• Develop and implement individualized PT care plans.
• Revise care plans in collaboration with nursing and the care team.
• Educate clients and families on rehabilitative care and home exercise.
• Document observations and interventions in the EMR.
