Qualifications

• Current state licensure as a Physical Therapist in good standing.
• Home Health clinical experience preferred.
• Strong clinical reasoning and decision-making skills.
• Excellent interpersonal and communication skills with residents and families.

Responsibilities

• Evaluate residents' physical abilities and rehabilitation needs.
• Develop and implement personalized treatment plans.
• Provide one-on-one PT sessions and use multiple modalities.
• Monitor progress and adjust plans to maximize outcomes.
• Document evaluations, progress, and discharge summaries per policies.
• Collaborate with care team to coordinate services.
• Educate residents and families on exercises and safety.
• Participate in care conferences and in-service trainings.
• Maintain professionalism and effective communication.
• Perform other duties as assigned.
