Job summary

Enhabit Home Health & Hospice is seeking a dedicated Physical Therapist to evaluate and treat patients in their homes. You will use physical modalities to improve mobility and independence, following physician orders and an established care plan under branch leadership.

The role requires active state licensure as a Physical Therapist, a valid driver’s license, and reliable transportation to support patient visits across assigned territories.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Help patients improve mobility and function and independence in self-care and daily living skills.
• Deliver skilled care under physician orders and plan of care, with supervision of Branch Director.
• Participate in specialty programs such as Balance and Fall Prevention, Spine Safety, and joint replacement programs.
