Qualifications

• Valid Physical Therapy license in the state of practice.
• 1+ years of physical therapy experience, preferably in home health.
• Excellent communication and interpersonal skills with patients and families.
• Ability to prioritize tasks and adapt to changing priorities in a fast-paced environment.
• Ability to build rapport with patients through compassion and care.

Responsibilities

• Conduct thorough evaluations of patients' physical abilities and needs to develop individualized therapy plans.
• Develop and implement treatment programs to improve mobility, strength, flexibility, and functional performance.
• Utilize exercises, manual therapy, modalities, and assistive devices to address impairments and goals.
• Educate patients and families on exercises, home programs, and recovery strategies.
• Monitor and document patient progress and changes, providing reports to the supervising physicians.
• Collaborate with physicians, nurses, and other healthcare professionals for coordinated care.
• Advise on assistive devices and home modifications to improve independence.
• Maintain accurate patient records compliant with standards and regulations.
• Participate in care plan development and team conferences to optimize outcomes.
