Job summary

First Senior Home Health seeks a dedicated Physical Therapist to provide skilled PT services in patients' homes in Clearwater, FL. The role includes evaluating needs, developing care plans, and collaborating with the care team to optimize patient outcomes.

You will perform OASIS assessments, supervise aides, and document progress while coordinating with physicians and family members to ensure safe, effective home-based care.

Qualifications

• Successful completion of a Physical Therapist education program approved by CAPTE or equivalent.
• Must be licensed or registered by the state of Florida.
• Must have at least one year of institutional/home health experience.
• Must have a criminal background check and current CPR certification (online not accepted).

Responsibilities

• Provide skilled Physical Therapy services in patients' homes, intermittently, per physician orders.
• Conduct initial and ongoing assessments including OASIS.
• Develop and implement interdisciplinary care plans for physical therapy within the home health setting.
• Evaluate patients with diagnostic and prognostic tests of muscles, nerves, joints, and function.
• Supervise Home Health Aides and PT Assistants as required by policy.
• Document treatment and patient responses; communicate with physician at least every 30 days.
