Job summary

Clearwater is seeking a licensed Physical Therapist to deliver skilled services in patients' homes under physician orders. You will perform comprehensive assessments, contribute to the patient care plan, and educate families and patients on PT programs and mobility devices.

Documentation and regular physician communication are required. Candidates must hold Florida licensure, CPR certification, and have home health experience preferred.

Qualifications

• Licensed Physical Therapist in Florida.
• CAPTE-accredited PT education program completed.
• Florida license to practice physical therapy.
• Current CPR certification.

Responsibilities

• Provide skilled physical therapy services in patients' homes on an intermittent basis as ordered by physicians.
• Perform initial and ongoing assessments, including OASIS as applicable.
• Develop and implement interdisciplinary patient care plans with the team.
• Instruct and supervise families and patients in PT programs and use of devices.
• Maintain timely progress notes and communicate updates to physicians at least every 30 days.
• Supervise Physical Therapy Assistants and Home Health Aides per agency policy.
