Job summary

Westhill Newnan Crossing seeks a licensed Physical Therapist to provide intermittent, in-home PT services across patient cases in Clearwater and surrounding areas. You will perform initial and ongoing assessments, contribute to interdisciplinary care plans, and educate patients and families on therapy programs.

The role requires Florida licensure, CAPTE-accredited education, and at least one year of home health experience, with CPR certification and strong English communication skills.

Qualifications

• Must have completed a CAPTE-accredited physical therapist education program and be licensed in Florida.
• Candidate should have at least one year of institutional/home health experience.
• Current CPR certification required; online certification not accepted.

Responsibilities

• Provide intermittent skilled PT services in patients' homes per physician orders.
• Perform initial and ongoing OASIS assessments and update care plans.
• Assess, and contribute to, interdisciplinary patient care plans.
• Perform diagnostic and prognostic tests for patient evaluation.
• Educate and supervise family and patients in therapy programs.
• Instruct on use of wheelchairs and assistive devices and arrange outpatient services when needed.
• Prepare patient condition assessments and periodic summaries.
• Supervise PT assistants and home health aides per policy.
• Document treatment progress and report changes to physicians.
