Job summary

First Senior Home Health Tampa seeks a Physical Therapy Assistant to deliver skilled PT services in patients' homes under supervision from a Registered PT. The role involves assisting in evaluation and developing a rehabilitative plan of care, and documenting progress for each visit.

You will prepare periodic summaries and discharge notes, guide patients and families through exercise programs, and collaborate with the PT and health team to ensure quality home care delivery.

Qualifications

• Must be licensed as a Physical Therapist Assistant in Florida.
• Must have current CPR certification.
• Must pass a criminal background check.
• Must be able to drive to patient homes.

Responsibilities

• Provide skilled Physical Therapy services to patients in their homes under the direction of the PT.
• Assist in evaluation and development of the rehabilitation plan of care and periodic re-evaluation.
• Prepare daily notes and progress summaries; ensure notes are submitted within 48 hours.
• Instruct patients and families in prescribed exercises and use of self-help devices.
• Collaborate with the PT and other health team members during discharge planning.
