Job summary

First Senior Home Health in Tampa, FL is seeking a Home Health Physical Therapist to deliver skilled PT in patients’ homes, perform comprehensive assessments, and contribute to interdisciplinary care plans.

You will teach and supervise patients, families, and aides, document progress, and participate in discharge planning. A FL license, CAPTE-approved program, CPR certification, and background check are required.

Qualifications

• CAPTE-approved PT education program completed.
• Must be licensed in Florida.
• At least one year of institutional/home health experience.
• Criminal background check required.
• Current CPR certification required (online certification not accepted).

Responsibilities

• Provide skilled Physical Therapy services in patients' homes per physician orders.
• Perform initial and ongoing assessments, including OASIS where applicable.
• Contribute to development and implementation of the care plan with the team.
• Assist physician in evaluating patients with tests for function and prognosis.
• Educate and supervise family and patient in the PT program.
• Instruct on use of mobility devices and prosthetics as needed.
• Document progress and discharge summaries within 48 hours of visits.
• Supervise Home Health Aide and PT Assistants as per policy.
• Participate in discharge planning and case conferences.
