Job summary

First Senior Home Health Clearwater is seeking a licensed Physical Therapist Assistant to deliver skilled PT services in patients’ homes under PT supervision in Clearwater, FL. This role involves patient assessment support, plan development, and family instruction.

Responsibilities include documenting notes, assisting in re-evaluation, and coordinating with the PT and nursing team to ensure quality home health care. Valid Florida PTA license and CPR are required.

Qualifications

• Licensed or eligible as a Florida Physical Therapist Assistant.
• Graduate of an approved PTA curriculum; must pass the national PTA exam.
• Must be licensed by the State of Florida as a PTA; background check required.
• Current CPR certification is required; online CPR not accepted.
• Must be able to drive to patients’ homes; good English communication.

Responsibilities

• Provide skilled physical therapy services as delegated by the Registered PT.
• Assist in evaluation and development of the rehabilitation plan of care and periodic re-evaluation.
• Prepare and submit daily progress notes within 48 hours; assist in discharge summaries.
• Instruct patients and families in exercises and use of home exercise devices.
• Collaborate with PT, supervise Home Health Aides, and attend staff meetings.
