Job summary

Senior Care Solutions in Largo, FL is seeking a compassionate Physical Therapist to perform home health services for patients in their homes. The PT will evaluate, treat, and educate patients and caregivers, develop plans of care, and coordinate with an interdisciplinary team.

Qualifications

Must hold a current Florida Physical Therapist license.
CPR certification current.
Home health experience preferred.
Strong clinical assessment and documentation skills.
Ability to travel within service area.

Responsibilities

Provide skilled physical therapy evaluations and treatments in patients' homes.
Develop individualized treatment plans based on patient needs and physician orders.
Perform assessments and establish appropriate goals for each patient.
Provide therapeutic exercises, mobility training, gait training, balance activities.
Educate patients and caregivers regarding treatment plans and home exercise programs.
Monitor patient progress and modify treatment plans as needed.
Coordinate with RNs, OTs, STs, MSWs and other team members.
Complete clinical documentation accurately within required timeframes.
Participate in case conferences and care coordination.
Maintain compliance with Medicare, state, and agency requirements.
Provide exceptional patient care while maintaining professional standards.
