Job summary

BrightStar Care is seeking a licensed Physical Therapist (PT) to join our home health team serving Leominster, MA. This part-time / per diem role offers meaningful work helping patients regain strength, balance, and independence in their own homes.

Responsibilities include in-home PT evaluations, personalized care plans, and collaboration with a multidisciplinary team. A MA PT license, DPT or MPT, and CPR certification are required; new grads welcome and mileage reimbursement provided.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification.
• 1+ year of experience preferred (new grads welcome)

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
