Job summary

Peace and Harmony Homecare LLC seeks a licensed Physical Therapist to provide professional PT services in patients' homes, collaborating with primary care physicians and the health care team. You will educate patients and families, assess functional status, and develop and monitor Plans of Care, with ongoing communication to the care team.

Responsibilities include home safety assessments, documenting assessments within required timelines, and ensuring compliance with licensure and CPR

Qualifications

• Successful completion and graduation from a CAPTE-approved PT program or equivalent
• Current, active license to practice Physical Therapy in the US
• Minimum of one year experience as a Physical Therapist
• Criminal background check and current CPR certification
• Equivalency criteria for international-trained PTs (CAPTE-equivalent or WPT equivalent)

Responsibilities

• Receives intake referral information and prepares PT paperwork/tools for visits
• Obtains medical history from patient and family
• Performs PT assessment and documents results
• Performs home safety and environmental assessment
• Develops and revises a PT Plan of Care with physician approval
• Supervises Home Health Aide when PT is the skilled discipline
• Communicates progress with patient, family, and physician
• Submits OASIS assessments/documentation within 48 hours of visit
• Performs clinical record reviews with Nursing Supervisor
• Participates in staff meetings and professional development
• Ensures licensure is maintained and seeks educational opportunities
