United Home Health Care is seeking a compassionate Physical Therapist to provide one-on-one care in patients' homes. You will conduct initial evaluations and develop personalized plans of care focused on mobility and safety.

Collaborate with physicians, occupational therapists, and nursing staff to support full recovery. Maintain accurate EMR documentation (OASIS a major plus) and teach home exercise programs to patients and families.

Formación

• Valid State License in Physical Therapy (PT).
• Home Health experience preferred; training available for qualified transitions.
• Self-starter with ability to manage own schedule and travel to patient homes.
• Strong interpersonal skills to build rapport with patients and families.
• Valid driver's license, reliable transportation, and auto insurance.

Responsabilidades

• Conduct initial evaluations and develop personalized plans of care focused on mobility and safety.
• Provide high-quality manual therapy, therapeutic exercise, and gait training in the home setting.
• Coordinate with physicians, OTs, and nursing staff for holistic patient care.
• Maintain timely clinical records in an EMR (OASIS experience a major plus).
• Educate patients and families on home exercise programs and fall prevention.
