Job summary

Peace and Harmony Homecare LLC is seeking a licensed Physical Therapist to provide professional PT services in patients' homes in accordance with practice standards. You will collaborate with primary care physicians and educate patients and families on therapy plans.

The role requires CAPTE-approved education, a current US PT license, and a minimum of one year of PT experience, plus CPR certification and a background check.

Qualifications

• CAPTE-approved PT program (or equivalent international program deemed equivalent)
• Active PT license to practice in the US
• Minimum 1 year of PT experience
• Criminal background check and CPR certification

Responsibilities

• Receive intake referral information and prepare PT paperwork/tools for the visit
• Obtain medical history from patient/family
• Perform PT assessment and document findings
• Conduct home safety and environmental assessment
• Develop and submit PT Plan of Care and revise every 30 days
• Functionally reassess patient for ongoing therapy
• Evaluate medical necessity for other services
• Assess ADL performance and assistive devices
• Supervise Home Health Aide when PT is sole skilled discipline
• Document supervision and maintain communication with patient/family
• Communicate with physician to obtain treatment modalities
• Submit OASIS assessments within 48 hours of visits
• Participate in committee work and staff meetings

Education

CAPTE-approved PT program

Current US Physical Therapy license

Minimum 1 year PT experience

Criminal background check & CPR certification
